第一條 為協助提升癌症診療品質，並依據本醫院組織規程第四十條規定，成立「癌症醫療品質管理委<br />
員會」(以下簡稱本委員會)。<br />
第二條 本委員會之執掌如下：<br />
一、提昇本醫院癌症醫療品質。<br />
二、推動本醫院癌症相關計畫、及研究之擬定、執行、評估及監測。<br />
三、推動及整合跨科室癌症醫療服務。<br />
四、推動各臨床專科進行癌症臨床試驗研究，並整合試驗研究結果。<br />
五、督導並監測本醫院癌症資料庫。<br />
六、建立院內癌症醫療同儕審查機制，以評估癌症診療之照護品質<br />
七、定期對癌症病人治療成果進行檢討與分析，並公佈癌症診療與照護報告<br />
第三條 本委員會置委員13~17人、幹事1~3人，由院長聘任之，召集人一人及副召集人一人由委員兼任<br />
之。<br />
前項委員及幹事由院長就癌症醫療相關專業人員中遴選聘任之，聘期一年，聘期屆滿，得連聘<br />
連任之。<br />
第四條 本委員會得依不同癌症照護需要成立各癌症診療照護團隊，並自委員中遴選團隊召集人一人，<br />
有關各癌症診療照護團隊任務與工作項目另訂之。<br />
第五條 本委員會之組織採功能分組，依委員會職掌區分為策略發展小組、醫療團隊運作小組、癌症登<br />
記小組、品質監測及資訊發展小組，其中醫療團隊運作小組依癌症特性分為十九個癌症診療照<br />
護團隊,分別為：乳癌團隊、口腔癌團隊、肺癌團隊、肝膽胰癌團隊、大腸直腸癌團隊、婦<br />
癌團隊、皮膚癌團隊、泌尿系癌團隊、小兒腫瘤團隊、胃癌團隊、食道癌團隊、淋巴癌團隊、<br />
頭頸癌團隊、甲狀腺癌團隊、腦腫瘤團隊、骨骼及軟組織癌團隊、局部癌化學治療團隊、癌<br />
症疼痛團隊、安寧療護團隊。<br />
第六條 各功能分組之任務如下：<br />
一、策略發展小組任務：<br />
1.規劃、督導及評估院內各項癌症相關計畫。<br />
2.針對癌症計畫訂定年度工作目標、優先順序、執行策略和相關機制。<br />
3.協助執行單位推動相關政策，並監督執行單位之工作成效，修正相關工作目標及機制。<br />
二、醫療團隊運作小組任務：<br />
1.監測本院癌症診療照護團隊運作狀況。<br />
2.協助癌症診療照護團隊任務之執行。<br />
三、癌症登記小組任務：<br />
1.督導癌症資料庫管理。<br />
2.監測癌症資料庫登錄正確性、完整性及申報之完成率、即時率。<br />
3.協助癌症資料庫統計分析。<br />
四、品質監測及資訊發展小組任務：<br />
1.建立院內同儕審查機制，評估癌症診療與照護品質。<br />
2.負責建立癌症病歷複閱及癌症登錄資料庫癌症分期之審閱機制。<br />
3.依據執行單位之癌症資訊發展需求，督導系統之建立。<br />
第七條 本委員會每二個月定期召開會議一次，必要時得召開臨時會議，會議召開時得邀請相關人<br />
員列席說明或報告。<br />
第八條 本辦法經本醫院院務會議審議通過，陳請院長核定後，自公布日起實施，修正時亦同。<br />
