</span><span style='font-family:標楷體;color:black'>本學院各級教師每五年須由各級教評會依本施行細則實施教學、研究、服務及輔導評估。教師未依規定完成評估作業者，該學年度評估視為未通過。教師評估未通過者，次年起每年應接受再評估，並應由本學院或教師發展暨教學資源中心予以輔導協助，直到再評估通過時止。教師評估未通過期間，自於下一學年度起不予晉級晉薪，且不得在外兼職、兼課暨申請休假研究或國內外進修。</span></p>

第一條        高雄醫學大學健康科學院(以下簡稱本學院)為推行教師評估，提升教師教學、研究、輔導與服務水準，依據本校教師評估準則第十條規定，訂定本施行細則。

第二條        凡本學院專任教師應由各級教師評審委員會辦理評估事宜。兼任教師評估辦法，得由各系(所)教師評審委員會訂定其評量辦法，逐級核備。

第三條        本學院每年八月初依人事室及教務處所提供受評教師名單，於十二月底前完成初、複審評估作業。校教評會應於第一學期結束前完成決審作業並將評估結果送交本學院作為續聘之參考依據。

第四條        本校教師評估準則實施日(90.02.09)後聘任之副教授(含)以下教師，應於新聘到任後一定年限內(講師於到任後六年內，助理教授於到任後七年內，副教授於到任後八年內)通過上一等級之教師升等。

於前項年限內未能升等者，除有延長升等年限之事由外，應由系(所)教評會於該教師聘期屆滿前六個月，提出教師不續聘案經三級教評會審議並報教育部核准後，予以不續聘。

第五條        本學院各級教師每五年須由各級教評會依本施行細則實施教學、研究、服務及輔導評估。教師未依規定完成評估作業者，該學年度評估視為未通過。教師評估未通過者，次年起每年應接受再評估，並應由本學院或教師發展暨教學資源中心予以輔導協助，直到再評估通過時止。教師評估未通過期間，自於下一學年度起不予晉級晉薪，且不得在外兼職、兼課暨申請休假研究或國內外進修。

教師連續三年再評估結果為未通過者，除有延長評估年限之事由外，應由系(所)教評會於該教師聘期屆滿前六個月，提出教師不續聘案經各級教評會審議通過，依程序辦理教師不續聘。

教師通過升等者，自其升等後重新起算評估年限。

第六條        本學院教師評估指標包含「教學」、「研究」及「服務與輔導」三大指標，每一指標總分為100分。各指標之評估項目及計分標準如下:

一、教學：含教學出勤、教學評量、教師成長與教學特殊表現，總積分以100分為上限。

(一) 教學出勤：

依本校「教師基本授課時數核計辦法」列計。每學期教學出勤積分=教師每週平均授課時數（含兼任行政職務減授時數）/系、所平均授課時數或本校教學時數比值*6分。惟採系、所平均授課時數或本校教學時數應由系、所統一規範。每學年教學出勤積分以15分為上限，五年總積分以75分為上限。教學出勤中之正課教學時數之計算:修課學生人數六十名以下者依實際上課鐘點數計算，六十一至八十名者依1.2倍計算，八十一至一百二十名者依1.5倍計算，一百二十一名以上者依2倍計算，外籍學生教學者依2倍計算。

(二) 教學評量

1.         以最近國科會生物處研究人員近五年研究表現指標（RPI值）予以評核。但聘兼主治醫師者，以研究表現指標乘以1.25核計。附設醫院派支援相關醫療機構者或經校方核定赴校外全職進修者，其年限得不列入計算。

4.        教學研究、行政及資源整合單位組長、秘書、研究所班主任、副系主任、附設中和紀念醫院臨床部科主任、醫務秘書、秘書及部主任：每學年核給9.0分。

5.        學科主任、院級中心主任、附設醫院臨床暨醫療相關單位科、室主任、部副主任、本校受委託經營之醫療事業臨床科主任、醫務秘書：每學年核給6.0分。

6.        行政教師、附設中和紀念醫院臨床暨醫療相關單位室副主任、護理長、督導、各委員會召集人或主任委員或總幹事或執行秘書、院級中心組長：每學年核給3.0分。

第七條  本學院教師評估分為綜合型、教學型及研究型教師。各類型教師之「教學」、「研究」及「服務與輔導」三大指標所佔權重總和為100%, 惟每一大指標所佔權重不得低於20%。

選擇綜合型、教學型及研究型教師應符合下列條件：

一、綜合型：本校專任教師均得選擇綜合型。

二、教學型：教學五年總分達90分以上，且於評估年限內曾榮獲1次校教學優良教師始得選擇「教學型」。本學院教學型教師比例以不超過8%之教師為上限。

三、研究型：研究五年總分達90分以上，且五年內有四件(含)以上由中研院、國衛院或行政院所屬機關補助之研究計畫。

第十條  教師如有下列情事得延長升等年限或評估年限，惟每次延長年限為一年，至多以二年為限。

一、   因留職留(停)薪、懷孕、生產或哺育幼兒、罹患重症或其他重大事由，檢具證明經所屬系(所)及本學院教評會審議通過，並報校教評會核備者。

二、   因校、院、系(所)務發展需要且屬教學型教師，得由各系(所)主管提報專案延長年限，經院、校教評會審議通過者。

第十一條  本細則若有未盡事宜，悉依本校教師評估準則及相關規定辦理。

第十二條  本細則經院務會議通過及教務會議核備，陳請校長核定後，自公布日起施行，修正時亦同。
